Springfield - Greene County Health Department


The health
department divides food service code violations into two categories -- critical
and noncritical.


Critical
violations
are more likely than other violations to contribute
to food contamination, illness and/or injury.


Noncritical violations, although important to the overall food
hygiene of the establishment, do not necessarily present an immediate threat to
food contamination, illness and/or injury.


A
report for each establishment that is inspected shows the number of critical
violations, along with a short statement as to the nature of each violation and
what should be done to correct it. The report also shows the number of
noncritical violations but doesn't list the nature of those violations.



AMIGO'S MEXICAN RESTAURANT

Critical Violations Found: 2

* Meat on make table was not being properly cold held: it was 55 degrees (Meat
was put on ice.)

* Staff was date-marking using 10-day use-by dates (Product must be used in
seven days; there were no items older than seven days in the inventory but
discard date must be changed to today's date.)

Inspector placed restaurant on risk control plan for food temperatures at this
time, provided cooling procedure training, cooling logs and refrigerator logs.

* Foods were improperly cooled: cheese sauce cooked Saturday was still 52
degrees and refried beans were still 47 degrees. (Cooked potentially hazardous
food must be cooled from 135 degrees to  70 degrees within two hours, then
from 70 degrees to 41 degrees within 4 hours; food was discarded.)

* Raw steak was 47 degrees and cooked chicken was  61 degrees in south
cooler (Potentially hazardous cold foods must be held at 41 degrees or below; food
was discarded.)

5/19/09 Reinspection

Establishment was closed due to failure to comply with risk control plan on
proper cooling procedures.

* Foods cooked yesterday and cooled were still 60 degrees and 66 degrees in
cooler (Food was discarded.)

* Cooked chili rellenos were in a broken refrigerator and not held at 41 degrees
or below (Food was discarded.)

5/21/09 Reinspection

Critical items were corrected; staff was following proper cooling procedures and
all refrigeration units were maintaining proper temperatures; application for
reinstatement of food service permit was accepted and it was approved to
re-open.

cold-holding (corrected by voluntary discard of affected food product)

* Several potentially hazardous foods on salad bar were 54-65 degrees (Staff
must maintain foods at safe, cold-holding temperature of 41 degrees or below; corrected
by voluntary discard of affected foods.)

* Sanitizing solution in wiping cloth buckets was 10ppm chlorine (Staff must
maintain at 50-100ppm chlorine to effectively sanitize food contact surfaces; corrected
by remaking solution to proper concentration.)

* Many fruit flies were on doughnuts in dry good storage (corrected by discard
of food and adequately covering other food products)

Inspector provided handouts in Chinese language about date-marking, reheating,
and proper cooling methods and requirements.

* There was a failure to demonstrate knowledge about proper cooling procedures,
reheat temperatures, and date marking

* Raw chicken was stored over the top of cooked chicken in the east room
refrigerator on the north wall (corrected)

* Staff was using improper cooling method: chicken broth cooked this morning was
82 degrees at 3 p.m. (discarded)

* No date marks were on any ready-to-eat potentially hazardous foods like cooked
chicken, rice, egg rolls, soup, etc.

* Potentially hazardous, ready-to-eat chicken in the walk-in cooler was not
properly date labeled with a seven-day discard date (Once prepared, staff must
date these food items with a seven-day discard date.)

* Dish machine was sanitizing at 0ppm (It must maintain between 50-100ppm
concentration of sanitizer for sanitizing cycle; staff must use the three-vat
sink to manually wash, rinse, and sanitize all dishes until the dish machine has
been repaired; dish machine may not be used until it's repaired.)

Inspector educated staff on proper storage of personal items in areas away from
food products/prep areas; advised that sanitizing of dishes must be done every
time, as part of cleaning process.

* Cream cheese was stored on ice at 61 degrees (Staff must maintain potentially
hazardous foods at 41 degrees or below for safe, cold-holding; repeat violation;
corrected by voluntary discard.)

* French toast was stored out at room temperature without time as control
procedure properly implemented (Staff must label potentially hazardous foods
with make/discard time, implement & post written policy and discard product
within 4 hours after taking from cold-holding.)

* Unlabeled bottle of chemical was under sink (Staff must label all chemicals
with common household name so as to easily identify contents of container; repeat
violation; corrected immediately.)

were on site; -Education was issued to employees through posting signs on proper
hand washing procedures; -Foods in walk-in cooler were at safe, cold-holding of
38 degrees; -Foods were properly date-marked in coolers; -Food contact surfaces
were clean to sight and touch; -Leaks at 3-vat sinks and mop sink were remedied;
-Faucets were adjusted to a position, above flood level of sink, providing a
sufficient air gap; -Sanitizing solution was at 100ppm chlorine concentration; -Manager
will use time as control for various ready-to-eat, potentially hazardous foods
on cold bar and will also provide documentation and logs upon revisits.
