BOLIVAR, Mo. (KY3) -- Personal and financial information was stolen from Citizens Memorial Hospital. Officials say W2 tax forms were mistakenly given to a scammer, and now all employees are at risk.
CMH hasn't yet said how many of its workers this data breach impacts. They say everyone who works there and at all of the dozens of locations in the Ozarks could be impacted after what happened in one office.
Hospital officials say an employee got a phishing scam email asking for W2 information. They say the worker thought it was legitimate and sent them all the information last week. CMH says its employees found out it was a scam when the encrypted information was accessed by a third party. That is when they notified the FBI and the IRS. The information includes Social Security numbers and wages earned.
